Perjeta Ingredients and Usage
Perjeta contains pertuzumab, a humanised monoclonal antibody that targets the HER2 protein on breast cancer cells. Roche manufactures this biologic using recombinant DNA technology in Chinese Hamster Ovary cells. Each 14ml vial delivers 420mg of pertuzumab at a concentration of 30mg per millilitre.
Doctors prescribe Perjeta with trastuzumab and docetaxel for HER2 positive metastatic breast cancer in adults without prior anti HER2 therapy. The infusion also forms part of neoadjuvant regimens for locally advanced, inflammatory, or early stage breast cancer larger than 2cm or node positive. Perjeta belongs to the HER2 receptor antagonist class and supports adjuvant therapy for HER2 positive early breast cancer at high recurrence risk.
How Does Perjeta Work?
Perjeta works by binding to subdomain II of the HER2 receptor, the exact site where HER2 pairs with HER3 and other HER family proteins. This binding blocks ligand initiated dimerization, which is the trigger for downstream signalling through the PI3K AKT and MAPK pathways. By halting these growth signals, Perjeta starves HER2 positive tumour cells of the cues they need to proliferate.
Perjeta also triggers antibody dependent cellular cytotoxicity, recruiting immune cells to destroy HER2 expressing cancer cells directly. The complementary action explains why Perjeta works best alongside trastuzumab, which binds a different region of the same HER2 receptor. Clinical trials show this dual blockade improves median overall survival by around 16 months in HER2 positive metastatic breast cancer versus trastuzumab and docetaxel alone.
Perjeta Side Effects
Perjeta can cause infusion related reactions, gastrointestinal side effects, and hematologic toxicity when combined with chemotherapy. Most reactions emerge within the first cycles of treatment and require careful cardiac and blood count monitoring.

Warnings
Perjeta carries a boxed warning for left ventricular dysfunction, including symptomatic congestive heart failure during or after treatment.
Perjeta also carries a boxed warning for embryo fetal toxicity, with documented cases of fetal death and birth defects.
Patients with known hypersensitivity to pertuzumab or any excipient must not receive Perjeta due to the risk of anaphylaxis.
Evaluate left ventricular ejection fraction before starting Perjeta and at regular intervals during treatment, especially when given with trastuzumab.
Severe infusion reactions and anaphylaxis have occurred, so monitor patients during and for at least 60 minutes after the first infusion.
Women of reproductive age must use effective contraception during Perjeta therapy and for 7 months after the final dose.
Avoid concurrent anthracycline based chemotherapy, which raises the risk of cardiac toxicity when combined with Perjeta and trastuzumab.
Inform your doctor of any heart disease, hypertension, or prior chest radiotherapy before starting Perjeta treatment.
Discontinue Perjeta permanently if left ventricular ejection fraction declines and does not recover on retesting.
Perjeta Storage Conditions
Store Perjeta 420mg/14ml vials in a refrigerator between 2°C and 8°C inside the original carton, which protects the solution from light damage.
