Topotu 4mg/4ml Injection Ingredients and Usage

Topotu 4mg/4ml Injection contains topotecan, a topoisomerase I inhibitor used in cancer treatment. A topoisomerase I inhibitor is a chemotherapy drug that blocks an enzyme cancer cells need to copy their DNA. Topotecan is a semi synthetic derivative of camptothecin.

Topotu 4mg/4ml Injection treats metastatic ovarian cancer after initial or later chemotherapy fails. It also treats small cell lung cancer that returns after first line therapy. Combined with cisplatin, Topotu treats advanced, recurrent, or persistent cervical cancer.

How Does Topotu 4mg/4ml Injection Work?

Topotu 4mg/4ml Injection blocks topoisomerase I, the enzyme that relaxes coiled DNA during cell division. Topotecan binds the topoisomerase I and DNA complex and stops single strand DNA breaks from rejoining. This action lets the breaks build up while the cell copies its DNA.

During DNA synthesis these single strand breaks turn into permanent double strand breaks. Mammalian cells cannot repair this damage, so the cancer cell dies through apoptosis. Topotu therefore halts cancer cell growth and slows tumour spread.

Topotu 4mg/4ml Injection Side Effects

Topotu 4mg/4ml Injection causes side effects common to cytotoxic chemotherapy. Low blood counts are the most frequent and most serious effect.

• Neutropenia (low white blood cells)

• Thrombocytopenia (low platelets)

• Anaemia (low red blood cells)

• Nausea, vomiting, and diarrhoea

• Fatigue and weakness

• Hair loss (alopecia)

• Decreased appetite and abdominal pain

• Fever and infection risk

Warnings

• Topotu causes severe bone marrow suppression, its main dose limiting toxicity. Severe neutropenia can lead to infection, sepsis, and death.

• Doctors must not start Topotu in patients with a baseline neutrophil count below 1,500 cells per mm3. They check blood counts often throughout treatment.

• Topotu is contraindicated in patients with severe bone marrow depression or severe hypersensitivity to topotecan. Anaphylactoid reactions have occurred.

• Topotu can cause interstitial lung disease and neutropenic colitis, both potentially fatal. Doctors monitor for breathing problems and abdominal symptoms.

• Topotu causes embryo fetal harm and is contraindicated in pregnancy and breastfeeding. Both male and female patients must use effective contraception during treatment.

Store Topotu 4mg/4ml Injection at 2°C to 8°C in a refrigerator, protected from light in its original carton. Do not freeze the vial, as freezing damages the solution. Keep Topotu out of the reach of children, and let only trained hospital staff dilute and administer it.
