Hilvel Ingredients and Usage
The active ingredients are Sofosbuvir (a nucleotide analog NS5B polymerase inhibitor) and Velpatasvir (an NS5A inhibitor). One tablet is taken once daily with or without food for 12 weeks, providing a complete treatment course for most Hepatitis C patients.
Hilvel is clinically indicated for:
- Chronic Hepatitis C virus (HCV) infection in adults
- All HCV genotypes (1, 2, 3, 4, 5, and 6)
- Patients with compensated cirrhosis
- Treatment-naïve and treatment-experienced patients
- HIV-HCV co-infected patients
How Does Hilvel Work?
Hilvel provides comprehensive antiviral action through dual mechanism:
- NS5B Polymerase Inhibition: Sofosbuvir halts viral replication by acting as a chain terminator
- NS5A Protein Inhibition: Velpatasvir blocks viral assembly and secretion
- Pan-genotypic Coverage: Effective against all hepatitis C genotypes

Hilvel Side Effects and Warnings
Common side effects include headache, fatigue, nausea, asthenia, and insomnia. Most side effects are mild to moderate in severity.
Serious Considerations
- Hepatitis B Reactivation: Risk of HBV reactivation in co-infected patients - test for HBV before starting
- Bradycardia with Amiodarone: Serious symptomatic bradycardia may occur with concomitant amiodarone
- Drug Interactions: Significant interactions with rifampin, carbamazepine, and St. John's wort
- Renal Impairment: Safety not established in severe renal impairment
- Pregnancy: Use only if potential benefit justifies potential risk
