FCM Ingredients and Usage

The active ingredient is Ferric Carboxymaltose, a stable complex of ferric iron and carboxymaltose. It is administered as a single intravenous dose up to 1000mg, either as a slow injection or diluted infusion, under medical supervision. FCM is clinically indicated for:
  • Iron deficiency anemia when oral iron is ineffective or poorly tolerated
  • Iron deficiency in chronic kidney disease patients
  • Postpartum iron deficiency anemia
  • Preoperative anemia management
  • Inflammatory bowel disease with iron deficiency

How Does FCM Work?

FCM provides rapid iron repletion through controlled iron release:
  1. Targeted Iron Delivery: Provides iron in a stable complex that minimizes free iron toxicity
  2. Macrophage Uptake: Taken up by reticuloendothelial system macrophages
  3. Controlled Release: Slowly releases iron to transferrin for hemoglobin synthesis
  4. Rapid Hemoglobin Response: Significantly increases hemoglobin levels within 1-2 weeks

FCM Side Effects and Warnings

Common side effects include nausea, headache, dizziness, hypertension, and injection site reactions. Hypophosphatemia may occur but is usually transient.

Serious Considerations

  1. Hypersensitivity Reactions: May cause anaphylaxis - administer in monitored settings
  2. Hypotension: Can cause transient hypotension during or after administration
  3. Hypophosphatemia: May cause significant phosphate lowering requiring monitoring
  4. Oxidative Stress: Free iron may potentially increase oxidative stress
  5. Overload Risk: Avoid in patients with iron overload syndromes

FCM Storage Conditions

FCM Injection requires proper storage conditions:
  • Store below 25°C in original carton
  • Protect from light and freezing
  • Use immediately after opening
  • Do not use if solution is discolored or contains particles
  • Keep out of reach of children
