The active ingredient is Botulinum Toxin Type A, supplied as a lyophilized powder that must be reconstituted with preservative-free 0.9% Sodium Chloride solution immediately before injection.

BOTOX is clinically indicated for both cosmetic and therapeutic purposes:

  • Cosmetic: Temporary improvement in the appearance of moderate to severe glabellar lines (frown lines), canthal lines (crow’s feet), and forehead lines.
  • Therapeutic: Treatment of Chronic Migraine, primary axillary Hyperhidrosis, Cervical Dystonia, Blepharospasm, Hemifacial Spasm, and upper/lower limb Spasticity (e.g., post-stroke).
  • Urological: Treatment of overactive bladder and detrusor overactivity.

How Does BOTOX Work?

Botulinum Toxin Type A is a powerful neurotoxin that acts at the neuromuscular junction to interrupt the signaling pathway from nerve to muscle. The mechanism is highly localized and precise:

  1. Synaptosomal Binding: The toxin binds to specific receptors on the nerve terminal, where it is internalized.
  2. Inhibition of ACh Release: Once inside, the light chain of the toxin cleaves the SNARE protein SNAP-25. This protein is essential for the fusion of the vesicle containing the neurotransmitter Acetylcholine (ACh) with the nerve cell membrane.
  3. Paralysis/Relaxation: By preventing the release of ACh, the signal to contract the muscle is blocked. This results in localized flaccid muscle paralysis or relaxation, which reduces spasm, spasticity, or minimizes the muscle activity causing wrinkles.

BOTOX Side Effects and Warnings

BOTOX is generally well-tolerated when administered correctly, but its potent nature necessitates strict adherence to dosage and site selection.

Black Box Warnings and Critical Precautions:

  1. Distant Spread of Toxin Effect: The most severe risk. Effects of the toxin may spread from the injection site to other areas, causing symptoms of botulism, such as swallowing difficulties (dysphagia), breathing difficulties, and muscle weakness. This can be fatal and requires immediate medical attention.
  2. Dosage: Doses are NOT interchangeable between different Botulinum Toxin products (e.g., Botox vs. Dysport). Units are product-specific.
  3. Dysphagia: Particularly when treating cervical dystonia or administering large doses, patients may experience difficulty swallowing, leading to aspiration pneumonia.
  4. Contraindications: Should not be used in the presence of infection at the proposed injection site or in patients with certain neuromuscular disorders (e.g., Myasthenia Gravis, Lambert-Eaton Syndrome).

BOTOX 100 Units Storage Conditions

BOTOX requires strict cold-chain handling to maintain potency:

  • Store the unopened vials of lyophilized powder in a refrigerator at a temperature between 2°C and 8°C (36°F and 46°F). Do not freeze.
  • Reconstitution: Vials should be reconstituted gently with 0.9% non-preserved saline. Vigorous shaking can denature the protein.
  • Stability: Once reconstituted, the solution should preferably be used immediately, but may be stored in the refrigerator for up to 24 hours before use.
