Filatil Ingredients and Usage

The active ingredient is Filgrastim, a recombinant methionyl human granulocyte colony-stimulating factor. It is administered as a daily subcutaneous or intravenous injection, typically starting 24 hours after chemotherapy completion.

Filatil is clinically indicated for:

  • Chemotherapy-induced neutropenia prevention and treatment
  • Peripheral blood progenitor cell collection and transplantation
  • Severe chronic neutropenia management
  • Neutropenia associated with HIV infection
  • Stem cell mobilization for transplantation

How Does Filatil Work?

Filatil stimulates neutrophil production through targeted hematopoietic action:

  1. Bone Marrow Stimulation: Binds to G-CSF receptors on neutrophil progenitor cells
  2. Neutrophil Production: Stimulates proliferation and differentiation of neutrophil precursors
  3. Maturation Acceleration: Reduces neutrophil maturation time from 5 days to 1 day
  4. Functional Enhancement: Enhances phagocytic activity and antibody-dependent cell-mediated cytotoxicity

Filatil Side Effects and Warnings

Common side effects include bone pain, headache, fever, fatigue, and injection site reactions. Bone pain is the most frequently reported adverse effect.

Serious Considerations

  1. Splenic Rupture: Rare but fatal splenic rupture reported – evaluate left upper abdominal pain
  2. Acute Respiratory Distress: May cause serious respiratory distress syndrome
  3. Sickle Cell Crisis: Can precipitate sickle cell crises in patients with sickle cell disorders
  4. Glomerulonephritis: May cause glomerulonephritis – monitor urinalysis and renal function
  5. Leukocytosis: Extreme leukocytosis may occur – monitor complete blood counts regularly

Filatil Storage Conditions

Filatil Injection requires strict storage conditions:

  • Store at 2°C to 8°C in refrigerator
  • Do not freeze – protect from light
  • Avoid shaking the solution
  • Use within 24 hours if stored at room temperature
  • Do not use if solution is discolored or contains particles
