• Malignancies: Non-Hodgkin’s Lymphoma (NHL), Hodgkin’s Lymphoma, Multiple Myeloma, Acute and Chronic Leukemias, Mycosis Fungoides (Cutaneous T-cell Lymphoma), Neuroblastoma, and Retinoblastoma.
• Immunosuppression: Severe, progressive, or life-threatening autoimmune disorders (e.g., severe rheumatoid arthritis, Wegener’s granulomatosis).

How Does C Tx Work?

Cyclophosphamide is a prodrug, meaning it is inactive until metabolized by the liver. Its mechanism involves damaging the DNA of malignant cells:

1. Hepatic Activation: The drug is converted by hepatic cytochrome P450 enzymes, primarily to the active metabolite Phosphoramide Mustard.
2. DNA Cross-Linking: Phosphoramide Mustard forms covalent bonds (alkyl groups) with the DNA of rapidly dividing cells, creating DNA inter-strand cross-links.
3. Apoptosis: This irreversible DNA damage prevents the cell from replicating, triggering G2 phase arrest and subsequent programmed cell death (apoptosis).

Cyclophosphamide is associated with severe, dose-dependent toxicities, particularly affecting the bone marrow and bladder.

Black Box Warnings and Critical Precautions:

1. Myelosuppression (Low Blood Counts): Cyclophosphamide causes severe bone marrow suppression, leading to neutropenia, anemia, and thrombocytopenia. Frequent blood count monitoring is essential.
2. Hemorrhagic Cystitis: The drug can cause severe bladder inflammation and bleeding. This is prevented by vigorous hydration and the use of the uroprotective agent Mesna.
3. Cardiotoxicity: High doses can cause cardiotoxicity, including heart failure.
4. Secondary Malignancies: Long-term use increases the risk of secondary cancers, particularly bladder cancer and leukemia.
5. Infertility: Can cause permanent infertility in both men and women.
